ABSTRACT

The unprecedented year 2020 has passed. Under the shadow of the COVID-19 epidemic, the global political, social and economic patterns have changed dramatically. Due to the rapid control of epidemic spread in China, medical system soon returned to normal. Minimally invasive gastrointestinal surgery represented by laparoscopy has completely returned to normal level. In view of prevention and control of the epidemic, international communication and transportation have been greatly affected. Many academic conferences have been cancelled or changed to be held online. The epidemic has greatly accelerated the popularization of online transmission of minimally invasive surgery. However, there are some differentiation and differences in disease spectrum, surgical technology, minimally invasive technology, surgical equipment and instruments at home and abroad. In view of this special situation in 2020, this paper reviews and prospects the new hot spots and new developments of minimally invasive gastrointestinal surgery in terms of surgical technology platform, surgical technology and concept.Copyright © 2021 Chinese Medical Association

ABSTRACT

When my husband was diagnosed as stage 4 gastric cancer, I sincerely wanted to help him. I believed that somewhere in the world there must be a doctor who could cure him. Unfortunately, I did not know how to choose the right information. During the pandemic, the WHO called the flood of information about covid-19"info-demic", I feel that this is the same with cancer. Knowing how to choose information is increasingly a critical skill in life. I believe that when patients and doctors dialogue and think together about"why they think so"and"why they made this choice"based on the gastric cancer treatment guidelines for patients, shared decision-making and precision medicine come true. My husband came up with the name "KIBOUNOKAI"for our organization. Through the gastric cancer treatment guidelines for patients, patients and doctors will become discussion buddies with the same hope and become a force to promote science. It is my husband's legacy and my determination.

ABSTRACT

PURPOSES: The spread of coronavirus disease 2019 (COVID-19) has affected socioeconomic and healthcare systems in many countries. Accordingly, many individuals may have canceled their annual health-check programs, including esophagogastroduodenoscopy, which would have resulted in lower numbers of newly diagnosed patients with gastric cancer in comparison to other times. METHODS: Questionnaires were distributed to 62 hospitals every week from May 2020 to August 2020 (total 744) through mailing lists of the Stomach Cancer Study Group of the Japan Clinical Oncology Group. The number of patients with gastric cancer and hospital systems during the COVID-19 pandemic were surveyed. RESULTS: In total, 74% (551 out of 744) of the questionnaires were answered and analyzed. In early May, approximately 50% of hospitals had to restrict surgical slots due to the COVID-19 pandemic. However, they gradually loosened the restrictions thereafter. The number of gastrectomies was < 80% that of the same period in the previous year, and hospitals in Tokyo were seriously affected by a 50% decrease in the number of gastrectomies. CONCLUSIONS: The number of gastrectomies was lower than that in the previous year. Further multi-center follow-up studies are required to evaluate the long-term effects of COVID-19 on the clinical outcomes of patients with gastric cancer.

ABSTRACT

Gastric lipomas are very rare benign tumors. Only around 217 cases have been reported. Most gastric lipomas are found incidentally; however, larger neoplasms can be symptomatic. The presented 64 years old male with incidental finding of pyloric lipoma adds another example to the few documented in the literature. The patient had symptoms of breath shortness and lack of energy two months after COVID 19 pneumonia. A low dose CT scan with iodine contrast enhancement of the chest and upper abdomen led to the diagnose. The diagnosis of gastric lipoma can be achieved through diagnostic imaging or the combination of endoscopic techniques. Treatment can be carried out by endoscopy, robotic or classical surgery.
